What Is Broken Hill Community Options? Community Options is a Home and Community Care (HACC) program funded to provide case management and brokerage services to its target group Broken Hill Community Options is funded by the New South Wales and Commonwealth Governments and is under the management of the Broken Hill City Council. The state funding body responsible for Community Options is The Department of Ageing, Disability and Home Care (DADHC). Broken Hill Community Options Target GroupBroken Hill City Council Community Options target group is the frail aged and younger people with a disability with complex care needs who are at risk of premature admission to residential care. Services are provided to assist these people to remain living independently in their home. How are these services provided?Community Options uses a brokerage system to provide services. This means we contract staff from other service providers to supply the services to the client. Service providers are specially selected to complement the individual needs of each client. All service providers are required to provide the highest quality of service to all Community Options clients; this is monitored via our quality management system. Services Community Options can provide Community Options can offer a range of individualised services in a flexible manner to support clients living in the community, these include:
Case Management
Case management is provided by Community Options Case Workers. You’re appointed Case Worker assists you to; identify needs, goals and strategies coordinate appropriate services and supports systems access services by providing you with advocacy monitor and evaluate service effectiveness This is done by maintaining regular contact with their clients and client’s carers/advocates. Case Workers can act as a single point of contact for their clients for any relevant assistance they may require and a liaison between clients and service providers. Personal Care
Well trained staff provide assistance with hoist transfers, toileting, showering, grooming, and dressing in a sensitive and respectful manner. Staff can also provide standstand by assistance in which the client completes personal care tasks and staff remain nearnear by to provide assistance if required.
Shopping
Staff can accompany and assist clients with their shopping or can complete shopping on the client’s behalf.
Domestic Assistance Domestic assistance includes vacuuming, cleaning of bathroom, toilet and kitchen, dishwashing, dusting, mopping, sweeping, the changing of bed linen, and general house tidying.
Gardening
The aim of this service is to minimise the safety hazards in the gardens of our client’s homes. The service can include lawn mowing, pruning, raking, sweeping, and general yard maintenance.
Respite Care
Respite provides an opportunity for carers to have a break from their caring role. Community Options will provide professional staff to undertake the required care which enables the carer to have a rest and have some time for themselves.
Individual Transport Assistance
Community Options can provide a staff person to transport clients to medical appointments, accompany them throughout the appointment and transport them home.
LaundryLaundry service is the washing of clothing and essential items. This service can be provided by staff in the home or client’s items can be taken to a professional Laundromat.
