What are Community Aged Care Packages?Community Aged Care Packages are planned and coordinated packages of care to help older people remain lilving in their own homes. They are funded by the Australian Government to provide for the complex care needs of older people. Who is eligible?Frail older people living in their own homes in the Broken Hill Local Government Area and the Unincorporated Area, who have complex care needs and would qualify for admission to residential care, but wish to continue to live in the community. What help is available?Community Aged Care Packages are very flexible and designed to help with indivudual care needs. It is important to know that the services provided can change, as your care needs change. How can you access a Community Aged Care Package?To receive a package an Aged Care Assessment Team (ACAT) must assess you. These specialists will determine whether your needs can best be met by a Community Aged Care Package. The ACAT assessment is free. How is a Community Aged Care Package arranged?When accepted, a package of services will be tailored to meet your needs as assessed by the ACAT Team. We will discuss the options available for meeting your care needs and a plan of care will be agreed upon. How much will you have to pay?You can be asked to pay a fee for a Community Aged Care Package. For older people on the maximum basic rate of pension, fees must not exceed 17.5% of that pension. People on higher incomes may be asked to pay additional fees 9limited to 50% of any income above the maximum pension rate). However, no one will be denied a service they need based on an inability to pay fees.
